Client overview

Skinfinity Spa is a medical spa in Louisville, Kentucky, offering advanced body and facial treatments including injectables, laser treatments, body contouring, and aesthetic skincare. Their website launched in September 2024 with goals to drive treatment bookings, surface high-value services like neuromodulators and PRP therapy, and convert search-driven visitors into appointments through an integrated calendar and shop page.

Louisville is a competitive market for medical spa SEO. Several established spas with 5+ years of domain history already ranked for the most valuable local terms. A new domain entering this market needed a focused, technically clean foundation before any off-page work could compound. I joined the project in September 2024, the same week the site went live.

The challenges I inherited at week one

A new domain with no organic presence is one problem. A new domain with critical technical errors blocking indexation is a much bigger one. Skinfinity Spa had both. Within the first week of audit I documented the following blocking issues:

  • Redirect loops on the homepage and several treatment pages, causing crawlers to abandon the URLs before reading them.
  • Faulty canonical tags pointing the wrong way on category pages, telling Google to ignore the very pages we needed indexed.
  • Sitemap errors the XML sitemap submitted to Search Console contained URLs that returned 404, lowering crawl trust.
  • Filler placeholder content on multiple service pages generic stock paragraphs that contributed nothing to topical authority.
  • Only the homepage was indexed at the start of the campaign. Treatment pages, the shop, and the booking calendar were all invisible to search.
  • Local visibility was zero no Google Business Profile, no citations, no presence in Louisville-specific search results.
  • No content strategy aligned with how Louisville residents actually search for medical spa treatments.

The 5-step strategy I executed

Medical spa SEO has a specific order of operations. Get the technical foundation wrong and every off-page link you build is undermined. Get the local foundation wrong and you compete in the wrong city. Below is the exact sequence I followed for Skinfinity Spa, with the rationale for each step.

Technical SEO audit and fixes

Before any keyword research or content work, I ran a full technical audit using Screaming Frog SEO Spider and cross-checked findings against Google Search Console’s Coverage and Page Experience reports. The audit surfaced 23 distinct issues across crawlability, indexation, redirect chains, and structured data.

  • Resolved every redirect loop by tracing chains in Screaming Frog and replacing 302 chains with single 301 redirects.
  • Fixed canonical tags on category and treatment pages so Google indexes the right URLs.
  • Removed pagination errors caused by misconfigured archive pages.
  • Submitted a cleaned-up XML sitemap to Search Console and requested indexing on all priority URLs individually.
  • Removed filler content on every page that didn’t serve a search intent. Around 30% of the pages on the site were deleted or merged.
  • Tightened the internal link structure so PageRank flowed correctly from the homepage to the highest-value treatment pages.

Result: indexable pages grew from 1 (homepage only) to 28 properly-indexed pages within the first 3 weeks.

Local keyword research and intent mapping

With the site now crawlable, the next priority was identifying which keywords would actually drive bookings — not just traffic. Medical spa search behaviour in Louisville has clear patterns: high-intent local modifiers (“medspa near me”, “medspa Louisville”), treatment-specific queries (“Botox Louisville”, “laser hair removal Louisville KY”), and brand-adjacent comparisons (“best medspa in Louisville”).

  • Pulled keyword data using Ahrefs and Google Keyword Planner, filtered to Louisville geographic intent.
  • Mapped 47 priority keywords across 3 intent tiers: informational, commercial, and transactional.
  • Restructured the site architecture so each treatment had its own dedicated URL with clear keyword-to-URL mapping.
  • Optimised the homepage for broad local terms (e.g., “med spa in Louisville KY”).
  • Optimised the shop page for commercial-intent treatment package queries.
  • Created dedicated landing pages for the top 7 treatments, each mapped to a specific commercial keyword cluster.

On-page content Optimisation

Once architecture and mapping were settled, every page was rewritten for SEO and conversion. Medical spa content has to walk a careful line: it must satisfy YMYL (Your Money or Your Life) quality signals from Google, but also drive an actual treatment booking. Generic spa content fails on both.

  • Rewrote every page on the site, replacing filler with treatment-specific information including procedure descriptions, expected outcomes, recovery times, and pricing context.
  • Added clear FAQ sections to each treatment page, addressing the questions Louisville residents actually ask about each procedure.
  • Optimised meta titles and descriptions for click-through rate, using emotional triggers and treatment-specific value propositions.
  • Added internal links connecting treatments to relevant before-and-after content, FAQs, and booking pages.
  • Implemented MedicalBusiness and Service schema on relevant pages.
  • Added trust elements: practitioner credentials, treatment certifications, and clear before-and-after expectations.

Local SEO and business profile setup

Medical spa is a fundamentally local search category. 80% of bookings come from local pack rankings, Google Business Profile interactions, and “near me” queries. A flawless local profile setup is non-negotiable. I built Skinfinity’s local SEO foundation from zero.

  • Created and verified Google Business Profile with complete service area, hours, treatment list, and high-quality interior photographs.
  • Set up Bing Places for Business — often overlooked but accounts for 5-8% of search traffic in some niches.
  • Set up Apple Business Connect — increasingly important as Apple Maps and Siri direct bookings grow.
  • Added booking buttons across all three profiles, integrated directly with the website calendar to capture appointments without phone friction.
  • Optimised the GBP categories to surface in both “medical spa” and “day spa” Louisville searches.
  • Initial booking volume from GBP started at 2-3 per week within the first month and grew steadily as visibility increased.

Citations campaign and authority building

With on-page and local foundations in place, the final phase was off-page authority building through manual citations and high-quality directory placements. This is the off-page SEO discipline at the core of what we do at Off-PageSEO.com.

  • Built 100 manual citations across national directories (Yelp, BBB, Healthgrades, Vitals, RealSelf), local Louisville directories (Louisville.com, GoLouisville, Louisville Chamber of Commerce), and medical-spa-specific directories (American Med Spa Association, Spafinder, Spa Index).
  • Ensured 100% NAP (Name, Address, Phone) consistency across every single citation — Google’s local algorithm penalises even small variations.
  • Each citation built on a domain with verifiable organic traffic — no link farms, no shady directory networks.
  • Generated supporting backlinks where editorial opportunity existed, contributing to domain authority growth.
  • Documented every placement with screenshots and live URLs, allowing the client to verify the work independently.

Result: by the end of the 3-month campaign, Skinfinity Spa had risen from a brand-new domain with no local presence to a competitive position in Louisville medical spa search — visible in the local pack for multiple treatment-specific queries.

Tools used in this campaign

Every campaign I run uses paid, industry-standard tools — not free alternatives. The Skinfinity Spa campaign was executed with the following stack:

  • Ahrefs — keyword research, backlink analysis, competitor gap analysis
  • SEMrush — secondary keyword validation and search intent mapping
  • Screaming Frog SEO Spider — technical crawl audits
  • Google Search Console — indexation monitoring and performance tracking
  • Google Business Profile Insights — local visibility and interaction tracking
  • Google Analytics 4 — conversion and revenue attribution
  • PageSpeed Insights — Core Web Vitals monitoring
  • Schema App / Schema.org JSON-LD — structured data validation

Next steps and ongoing work

The first 3 months established the foundation. The next phase focuses on compounding the gains through content and off-page authority.

  • A one-year content calendar of 4-5 educational blog posts per month, each targeting a specific treatment FAQ or local search query.
  • Expanded backlink campaign focusing on editorial guest posts in beauty, wellness, and aesthetic medicine publications.
  • 50 additional citations targeting Kentucky-specific business directories.
  • Quarterly technical audits to maintain the clean foundation built in Phase 1.
  • Review generation strategy on Google Business Profile to reinforce local ranking signals.

Want similar results for your medical spa or local service business?

Skinfinity Spa’s results came from a disciplined, sequenced approach: technical foundation first, then local SEO, then off-page authority. If you operate a medical spa, dental practice, aesthetic clinic, or local service business and you want a similar trajectory, the same methodology applies.
